Albany, NY – Ascend Wellness MD, a concierge medical practice focused on personalized healthcare, celebrated their grand opening today with a ribbon cutting ceremony at their 59 N Pearl Street location. Joining Downtown Albany Business Improvement District Executive Director Georgette Steffens and Ascend’s Founder & Medical Director Dr. Jackcy Jacob were Mayor Kathy Sheehan, Assemblymember Patricia Fahy, Economic Development and Sustainability Coordinator for Albany County Executive's Office, Lucas Rogers; and property owner and Redburn Development Partners. “The patient-physician relationship used to be special and can be again,” Dr. Jacob says emphatically. Board-certified in pediatrics and internal medicine, Dr. Jacob has more than 12,000 hours of training and has been a physician for 17 years. Her frustration with the constraints of traditional healthcare inspired her to open Ascend Wellness MD, where she can devote time to truly understanding her patients. “In my practice, patients’ lived experiences can’t be fully understood in a rushed 15-minute clinic visit or a brief hospital ward interview. It takes time to really listen, ask questions and connect with people to understand where they come from and who they are today,” she says.  

Ascend Wellness MD rebranded from Reviva Wellness in 2023 to reflect its expanded services and mission of helping patients reach new heights in health and well-being.
